This form is to arrange a collection for infectious healthcare waste that requires specialist treatment/incineration only.

Please note: We are not able to collect sharps and other waste contaminated with cytotoxic or cytostatic medication.
Ad-hoc sharps container collections can be requested by the patient as and when required using our Ad-hoc Sharps Collections request form.
