Applying for Changing Places access

Please sign in or register for a MyAccount – Mansfield District Council, at www.mansfield.gov.uk/myaccount if you are resident to request a user account for the Changing Places facility. This will enable you to manage your account, update information, add documentation and track progress / receive updates.

Once you have an account you will also be able to access other council services or update your personal details.

Important Information

Only one fob for the facility per carer or user will be provided. Lost or damaged fobs must be reported to hldadmin@mansfield.gov.uk or by calling 01623 463073.

To be given access to the Four Seasons Shopping Centre Changing Place Facility, evidence of either:

  • Higher Rate Disability Living Allowance (Care or Mobility Component),
  • Personal Independent Payment,
  • Attendance Allowance,
  • or diagnosis letter.

In addition to:

  • Proof of ID for both the User and Carer (The name and address on the IDs should be consistent with the details provided in the application form)

must be provided with this form.

You will be asked to attach clear images of these documents to this form later.

Terms of Use

The Changing Place facility is not intended for independent use and the user would normally be accompanied by a carer. Only one fob to the facility per person or carer will be provided.

Changing Places are intended for disabled customers who require the assistance of a carer to use the facility. To use the Four Seasons Changing Place, users are required to register with Mansfield District Council to obtain a fob for access to the facility. Evidence of high rated Disability Living Allowance, Personal Independent Payment, Attendance Allowance or diagnosis letter must be provided before a fob can be issued. Fobs are registered to an individual and are not to be transferred between users. Misuse of the facility or fob may result in the fob being blocked and the user unable to access the facility.

This facility is not intended as a baby changing facility or a standard disabled toilet, there are alternative facilities at this location for these users.

As a carer using the facility you will need to satisfy yourself that the slings you are using are compatible with the hoist prior to use. Mansfield District Council does not supply slings for health and safety reasons. If there is any doubt as to the compatibility of slings, then the hoist should not be used. The hoist that is supplied in this facility is the Astor Bannerman A2b and comes with a loop system hanging bar.

Prior to using the facility, please use the protective paper provided and remove the paper after use, leaving the changing bench clean for the next user.

All soiled or disposable material should be placed in the sanitary bin provided. Please take all soiled clothing away and be considerate to other users by leaving the room clean and tidy.

As a carer or user, it is your responsibility to always ensure safety, especially when using the hoist. Under no circumstances should the weight limit of the equipment be exceeded. Hoist maximum weight is 31.5 stone (200kgs) and the changing bench maximum weight is 30 stone (190.5kgs).

This facility is not accessible by RADAR key due to the ease of availability of these keys, it’s location and the aspiration to maintain this facility to a high standard and prevent misuse by others.
