If you run a business which offers acupuncture, piercing, electrolysis, tattooing (semi-permanent and permanent) you must get a registration from us.
To apply for or amend your personal / premises registration, please send your completed application form to the Licensing Department at licensing@mansfield.gov.uk or by post to the below address. You will be contacted to make payment, once the application has been received.

Cosmetic Piercing Registration application forms
An application fee must be paid when you make your application.
Registration is required for both the person carrying out the treatment and the premises used. There is a separate fee for the premises and the person.
You will need to re-apply if you move premises.
We will confirm that we have received both your application and fee and arrange to carry out an inspection of your premises.
You must ensure that your procedures, equipment and facilities used are:
- safe
- hygienic
- prevent the spread of disease, and
- comply fully with the general duty of care required by the Health and Safety at Work etc. Act 1974 (opens in new window).
The registration lasts until you either cancel it or your business ceases to exist.
Make changes or replace a tattoo and skin piercing registration certificate
You can use this process to:
- transfer a premises registration certificate
- change the premises that you operate from.
- update the name of a registered practitioner
- update the name of your registered premises (change of trading name)
- replace a lost or stolen certificate

The fee for making changes to your name, business name you operate from or replacing a certificate is £14.00
You will be contacted to make payment, once the application has been received.
